Jacqueline Smith Obituary, CLEARFIELD COUNTY, Pa. — Jacqueline Smith, 51, of Clearfield, Pennsylvania, has died following a fatal UTV crash in Penn Township on Friday afternoon.
According to Pennsylvania State Police, the crash occurred in the 100 block of Keiser Road when a UTV carrying three people left the roadway and struck a tree. Smith was ejected from the vehicle during the collision and suffered fatal injuries. She was pronounced dead following the crash.
Police said none of the three occupants were wearing seat belts or helmets at the time of the incident. Investigators determined that the UTV was being operated under the influence of alcohol or drugs, according to the preliminary investigation.
Authorities identified 45-year-old Levi Smith of Sidman as the driver involved in the fatal crash. He is facing multiple criminal charges in connection with Jacqueline Smith’s death, including homicide by vehicle, aggravated assault by vehicle while driving under the influence, recklessly endangering another person and driving under the influence.
Online court records show that Levi Smith is also facing charges related to reckless driving, careless driving resulting in unintentional death, driving at an unsafe speed, driving in an unsafe manner under the influence and driving without a helmet. He is currently being held at the Clearfield County Prison, with a preliminary hearing scheduled for Wednesday.
The charges against Smith stem from the initial investigation and remain subject to court proceedings. Pennsylvania State Police continue to investigate the circumstances surrounding the UTV crash, including the events that led to the vehicle leaving the roadway.
